Reserve your medical appointment or social event today!

Information Needed to Schedule Transportation:

When you call to make transportation arrangements, please have the
following information ready for the Buckeye Transport Service Dispatcher:

  •  Your name, phone number and extension
  •  Your facility’s name and address
  • The patients full name as it appears on his/her ID Card
  • The patients home address (including County)
  • The patients social security number
  •  The patients Insurance ID number, county of origin and status of
    eligibility
  • The member’s date of birth
  • The member’s room number
  • An emergency contact person with phone number for the member
  • A destination contact person with phone number (at the location to
    which the member  is being transported)
  • Type of service required: ambulatory, or wheelchair
  • Any special needs the member may have such as a mental or
    physical disability or the need for an escort.

Canceling or Changing Transportation Arrangements:

If transportation arrangements need to be changed or cancelled, please
call dispatch as soon as possible at 330-784-7388.
